ADDITIONAL REQUIREMENTS FOR PHILIPPINE PASSPORT APPLICATION OF MINOR CHILDREN IN CERTAIN CASES:

If minor is not traveling with either parent or is traveling alone

  • Personal appearance of the mother is required if minor applicant is an illegitimate child.
  • Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion and relationship to the minor. If minor will be traveling alone, ASC must be executed by either parent, stating that his/her child will be traveling alone. If minor is illegitimate, mother should execute the ASC.
  • Original and photocopy of DSWD Clearance
    • There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.

If both parents are abroad:

  • Affidavit of Support and Consent (ASC) executed by either parent indicating the name of the traveling companion (authenticated by the nearest Philippine Embassy or Consulate General). If minor is illegitimate, mother should execute the ASC.
  • Special Power of Attorney (SPA) with an attached photocopy of either parent’s valid passport (authenticated by the nearest Philippine Embassy or Consulate General) authorizing a representative in assisting the child to apply for a passport. If minor is illegitimate, mother should execute the SPA.
  • Original and photocopy of DSWD Clearance
    • There is no need to secure a DSWD Clearance if the minor traveling abroad has parents who are in the Foreign Service or living abroad or are immigrants, provided he / she is holding a valid pass such as a dependent’s visa / pass / identification card or permanent resident visa / pass / identification card which serves as proof that he / she is living with parents abroad.
  • Proper ID of the duly authorized representative (Please refer to List of Acceptable IDs)

If minor is legitimated by subsequent marriage of parents:

  • Authenticated Birth Certificate from NSO must include annotation regarding new status as legitimated and the full name of the child

If minor is illegitimate but acknowledged by father:

  • Birth certificate from NSO reflecting surname of father with Affidavit of Acknowledgement and Consent to use the surname of father

Foundling:

  • Certificate of foundling authenticated by NSO
  • DSWD Clearance
  • Passport of the person who found the applicant
  • Letter of authority or endorsement from DSWD for the issuance of passport

Orphaned minor applicant:

  • Authenticated Death Certificates of parents from NSO
  • Court order awarding guardianship of the orphaned minor applicant or substitute parental authority under Article 214 & 216 of the Family Code
  • DSWD Clearance

Abandoned minor applicant:

  • Court order awarding guardianship of the abandoned minor applicant or substitute parental authority
  • DSWD Travel Clearance

Legally adopted:

  • Original and Certified True Copy (CTC) of NSO Birth Certificate before adoption
  • Original and Certified True Copy (CTC) of the NSO amended Birth Certificate after adoption
  • Certified True Copy (CTC) of the Court Decision or Order on Adoption and Certificate of Finality
  • DSWD clearance for minor applicant, if traveling with the person other than the adopting parents

    In case the applicant is for adoption by foreign parents:
    • Certified True Copy of the Court Decree of Abandonment of Child
    • NSO Death Certificate of the child’s parents or the Deed of Voluntary Commitment executed after the birth of the child
    • Endorsement of child to the Intercountry Adoption Board by the DSWD
    • Authenticated Birth or Foundling Certificate

Minor applicant whose parents' marriage are annulled or whose parents are divorced:

  • Court order awarding guardianship of the minor applicant or substitute parental authority
  • DSWD Travel Clearance
  • NSO Marriage Certificate with annotation on nullity or annulment decree

Minor applicant whose mother is likewise a minor:

  • Personal appearance of mother and maternal grandparent/s
  • NSO Birth Certificate of minor applicant and mother
  • Affidavit of Support and Consent executed by the maternal grandparent/s indicating the name of the traveling companion
  • DSWD Clearance if minor will be traveling with the person other than the maternal grandparent/s
  • Proof of identity of mother and maternal grandparent/s (Please refer to List of Acceptable IDs)


LIST OF ACCEPTABLE IDs FOR PHILIPPINE PASSPORT APPLICATION


DEPARTMENT OF FOREIGN AFFAIRS (DFA) LIST OF ACCEPTABLE IDs (At least 1 of the following):

  • Government-issued picture IDs such as the following:
    • Digitized SSS ID
    • Driver’s License
    • GSIS E-card
    • PRC ID
    • IBP ID
    • OWWA ID
    • Digitized BIR ID
    • Senior Citizen’s ID
  • Other acceptable picture IDs such as the following:
    • Old College ID
    • Alumni ID
    • Old Employment IDs
DFA may require supporting documents to prove identity of passport applicant -. 

LIST OF SUPPORTING DOCUMENTS (At least 3 of the following):


  • Voter’s ID
  • NSO Marriage Contract
  • Land Title
  • Seaman’s Book
  • Elementary or High School Form 137 or Transcript of Records with readable dry seal
  • Government Service Record
  • NBI Clearance
  • Police Clearance
  • Barangay Clearance
  • Old documents issued at least one year prior to date of application that show correct name, date and place of birth, picture and signature of applicant such as the following:
    • NSO Birth Certificate of child / children
    • SSS-E1 Form or Microfilmed Copy of SSS-E1 Form
    • Income Tax Return
    • Voter’s Certification, List of Voters and Voter’s Registration Record (for newly issued VRR, please attach receipt)
    • School Yearbook
    • Community Tax Certificate

How to Get Philippine Passports For Minor Children

My family and some of my friends asked me to write a post about how to get Philippine Passports for their babies or minor children. Yes, folks. In spite of your babies or children being so cute, adorable and small, they still need passports in order to travel abroad. It is a good thing that the Department of Foreign Affairs (DFA) has made passport application easy with its courtesy lane. For more details, you can opt to call the DFA passport appointment hotline.

So, please prepare the following requirements for your child’s passport application:

1. D
ownload the form from the DFA website or you could go personally to the nearest DFA office.

2. Accomplish the passport application form.

3. Original Birth Certificate from the National statistics office (NSO).

4. If parents are married, Marriage Certificate of parents on printed Security Paper (i.e, authenticated by the NSO). Please prepare two (2) sets: a) one  original and b) one photocopy.

5. Photocopy of valid passport of either parent (if minor is a legitimate child)/ of mother (if minor is an illegitimate child) or identification documents (click HERE for the list of acceptable IDs).


6. Document of identity with photo, if minor is 8-17 years old (for first time and renewal applicant) such as School ID or Form 137 with readable dry seal.

For minor applicants who never attended school, a Notarized Affidavit of Explanation executed by either parent (if minor is a legitimate child) / by mother (if minor is an illegitimate child) detailing the reasons why the child is not in school, is required.

7. Notarized Affidavit of Support and Consent to travel from either parent (if minor is a legitimate child) / from mother (if minor is an illegitimate child).

8. Original and photocopy of valid passport of the person traveling with the minor.

At the courtesy lane of DFA, you will be directed to the steps from submitting the document, paying for the passport fee, having your baby’s picture taken and for the passport delivery service.

The DFA courtesy lane are for the following:
  • Senior Citizen
  • permanent government employees 
  • families of permanent government employees 
  • pregnant women 
  • kids below one (1) year old.
Good news folk! You don't need an appointment unlike the regular passport application. And the whole process can be done in less than an hour!

Commission on Elections picks PICC for elections canvassing—The Philippine International Convention Center (PICC), the country’s biggest and most elegant events venue, was again chosen to be the site of the canvassing for the 2013 Philippine mid-term elections. The Commission on Elections (COMELEC) chose the PICC as the venue of the national canvassing of the National Board of Canvassers (NBOC) for 10 days after the elections that chose the next batch of Senators and local government officials and legislators. The Jag Origins fashion show/ seminar launches Jag's Holiday 2013 collection that featured stretch denim for men and ankle-cut jeans for women.

No less than Project Runway judge and Marie Claire creative director Nina Garcia served as the inaugural speaker of JAG Origins last Friday. We were ecstatic when we learned during her interview with Boy Abunda that Nina Garcia also served as fashion director of fashion magazine Elle for 17 years.
The well-attended Jag Origins fashion show also showcased the latest creations of Filipino designers Jerome Salaya Ang, Norman Noriega and Jeffrey Rogador.
Jag Origins fashion show/ seminar is envisioned to be an annual seminar of influence and inspiration that "spreads art, design and creative inspiration" among upcoming fashion designers.
I also saw several fashion designers as well as famous celebrities in the audience and some beauty queens also took part in the event, including previous Miss Universe delegates Janine Tugonon and Shamcey Supsup and 2013 Miss Universe-Philippines Ariella Arida.
